When you submit an appointment request we collect your name, phone number, email address, preferred date and time, the reason for your visit and any notes you add. We ask you not to include detailed medical information in the form.
How the information is used
Appointment requests are used only to contact you about your enquiry and to arrange an appointment. Clinical records created during treatment are kept separately, in the practice's clinical record system, in line with applicable professional and legal requirements.
WhatsApp and phone enquiries
Messages you send through WhatsApp are handled by WhatsApp under their own terms. Please avoid sending sensitive medical details over messaging.
Retention and access
Enquiry details are retained only for as long as needed to respond to you and to keep a record of your appointment. You may request access to, correction of, or deletion of the information we hold about you.
